Monday - Friday: 7:00 a.m. - 4:30 p.m. Case Management is
a customer focused process designed to achieve quality, cost effective
patient outcomes by maximizing the coordination of interdisciplinary
services throughout the continuum of care. The department compliment
is comprised of physicians, registered nurses, case managers,
and medical social workers. Case managers act as the primary liaison
among the patient, the patient's family, nursing staff, physicians,
and other professional staff.
